Probable cause: The pilot’s improper decision to take off from a lake in gusting wind conditions, which resulted in a loss of airplane control.

Probable cause: The pilot’s delayed decision to abort the takeoff and his failure to properly conduct a rejected takeoff, which resulted in a runway overrun.

Probable cause: The pilot’s failure to maintain adequate clearance and visual lookout while taxiing, which resulted in his airplane colliding with the stationary airplane.
