What reportedly happened: The mechanic who performed the last annual inspection on the Taylorcraft saw the 73-year-old pilot take the airplane out of the hangar. Asymmetrical flaps bring down Lancair

Aircraft: Lancair 320. Injuries: Minor Location: Spokane, Wash. Aircraft damage: Substantial.

What reportedly happened: The pilot initiated a go-around followed by a climbing left turn to re-enter the traffic pattern. He was unable to roll out of the turn. The bank angle continued to near 90° before he was able to maneuver the airplane close to the runway. The airplane ultimately stalled and crashed on the runway.

Flying after engine fire fatal for pilot

Aircraft: RV-9A. Injuries: 1 Fatal. Location: Oregon City, Ore. Aircraft damage: Destroyed.

What reportedly happened: A few days before the accident, the pilot noted that the RV-9A’s engine was operating erratically. The day before the accident it lost power while the pilot was taxiing. It took several attempts to restart the engine. When it did start, flames came out of the inlet port. A fire extinguisher was used to extinguish the fire.

What reportedly happened: The pilot departed from his private airstrip. He saw thunderstorms approaching the area and noted the winds were increasing, so he returned to the airstrip for landing. On final approach, he noticed that a gate had been left open and his miniature donkeys were crossing the runway.

Night IMC kills three

Aircraft: Piper Cherokee. Injuries: 3 Fatal. Location: North Myrtle Beach, S.C. Aircraft damage: Destroyed.

What reportedly happened: The pilot, who had logged 640 hours, received a one-hour “checkout” with a CFI on July 6, 2010, so that he could rent the airplane. He was supposed to fly again, solo, on July 12, to satisfy night currency requirements, however, he canceled that flight due to adverse weather. He rented the Cherokee on the day of the accident, with the stipulation that he return it by 8:30 p.m., which was the onset of night, due to his lack of night currency.
